HELP US MEET THE HEALTH AND SAFETY NEEDS OF YOUR CHILD

  • Students with Asthma should keep an INHALER in their backpack and/or in the Health Office.  
  • Students with Food/Insect/Latex Allergies should keep an EPIPEN in their backpack and/or in the Health Office. 
  • Parents of students with Asthma should also update the health office with physician orders; all students carrying an inhaler must have a doctor's order on file in the health office.
  • Update the school health office with any new medical diagnoses your child has or any new medications your child is taking.
  • Inform the school health office of any changes to your child's emergency card/contacts; including new phone numbers.
  • Update the school health office with  any known severe food allergies (such as tree nuts, peanuts, etc.) or other severe allergies (such as insect bites) your child has.  Please make sure the school health office has current health care provider orders for EPIPENS and BENADRYL.
  • Provide the school health office with EPIPENS and BENADRYL as ordered by your child's physician in the event of an emergency as Volcano Vista is NOT a "peanut free" school.

NO MEDICATION OR SUPPLIES CAN BE ACCEPTED WITHOUT A VALID ORDER SIGNED BY THE MEDICAL PROVIDER AND THE PARENT. APS school staff will no longer be able to facilitate completion of medication forms by the student's provider

 Medical providers are no longer accepting fax requests regarding orders to provide nursing care for your student.  It is critical that you  obtain these orders signed by the provider and parent, PRIOR to the beginning of the school year.  

ALERTS          EpiPens recalled over failure to activate:

Posted March 20, 2017 20:53:42 Four batches of life-saving anti-allergy EpiPens have been recalled because they may fail to activate or require increased force - with potentially deadly consequences. About 80,000 EpiPen 300 microgram adrenaline injection auto-injectors, that may contain a defective part, have been distributed worldwide.

The EPI pen recall has extended to the United States:

In the United States, the recall applies to 13 lots of both EpiPen and EpiPen Jr. auto-injectors distributed between December 17, 2015, and July 1, 2016. Patients can receive another EpiPen or an authorized generic version at their pharmacy, Mylan said. In the meantime, they should continue to carry and use their current EpiPen until they acquire a replacement.
