Registration Information
For the 2024-25 school year, if your child has a health condition or will need a medication or special diet at school, etc, please find the appropriate action plans/forms at the left column or in the virtual health office above.
Forms and medications can be turned in to the health office during registration, or on August 7 or later.
- Please also remember to sign the parent signature line.
- If your child is new to APS, we will need their shot record.

Medication and Supplies
Medication and/or supplies must be accompanied by a valid medical order signed by your student's health care provider and parent/guardian.
- Please find specific forms for HCP orders under the "Forms" section or in the Virtual Health Office.
